SMART EMPLOYEE TRANSITION AUTHORITY - Salary Packaging

The WA Health's Salary Packaging Arrangement for Salary Packaging Administration Services allows eligible employees the option to change (transition between) Paywise to Smart.

You will only be eligible to transition to Smart during the transition period, 1-28 February 2025. The Salary Packaging transition form must be completed and received by Smart by 11:59pm on the last day of the transition period.

Any requests to transition outside the transition period will not be processed.

I confirm that I wish to transition my salary packaging arrangements from Paywise to Smart effective no later than 1 April 2025.

I also authorise Paywise to release all my personal details held on their record to Smart.

Smart will pass your personal information on to your Agency's payroll section as part of the administration of your salary packaging. Your personal information will not be used for any other purpose or disclosed to any other third party, without your consent, unless authorised or required by law.
